The unique Environ® Surgical Roll-CIT® represents a revolutionary approach to anti-ageing through natural processes. Unlike laser resurfacing which destroys the tissues of the papillary and reticular dermis, treatment with the Environ® Surgical Roll-CIT® relies on a more physiological method of inducing production of collagen and elastin. Unlike the 1mm Environ® Medical Roll-CIT, treatment using 3mm Surgical Roll-CIT® must be conducted by a surgeon or trained surgical assistant in a sterilised environment. The advantage of the Surgical Roll-CIT®, however, is that it is a once-off treatment with significantly deeper penetration and is even more effective at softening deep lines and wrinkles and reducing burn damage, keloid scarring and pigmentation marks.

Product Description

PCI-needling using the Environ Surgical Roll-CIT® causes microscopic breaks in the blood vessels deep below the surface layer of the skin. The needle only penetrates through the epidermis and does not remove it, so the epidermis is only cleft and will rapidly heal.

This injury, minute as it might seem, does cause some temporary localised damage and bleeding. Percutaneous induction of collagen results from the natural response to wounding of the skin. Platelets are released that contribute to the forming of blood clots.

In turn, these platelets release a series of growth factors which promote the production of collagen and elastin in the skin. Each tiny wound goes through the three classic phases of wound healing; Inflammation, Proliferation (tissue formation), and Tissue remodelling.

The needle pricks penetrate about 1.5 mm into the dermis and start up the inflammation phase. Tiny blood vessels are ruptured so blood cells and serum enter the surrounding tissue. A complex chemical cascade determines the formation of a clot, vascular permeability, the attraction for leucocytes, and fibroblasts are recruited into the wounded area.

Platelets cause clotting and release chemotactic factors like Platelet Derived Growth Factor (PDGF), transforming growth factor, and Fibroblast growth factor (FGF) that initiate an invasion of other platelets, leucocytes and fibroblasts. Collagen type III is gradually replaced by Collagen I over a period of a year or more. This gives increased tensile strength. The metallo-proteinases are essential for the conversion process.

This happens at a microscopic level and the related chain of events should be aided wherever possible to promote sufficient increases in elastin and collagen production in order to tighten and smoothen the skin as much as possible.

The only proven way to further boost collagen production is to prepare the skin with vitamin A prior to the treatment and then maintain an enhanced state of collagen and elastin production by continuing the skin care regime thereafter. The version of vitamin A that is used could be the medical form, Retinoic acid (Tretinoin) or the precursors of retinoic acid, which are either retinol, retinyl palmitate/acetate or retinyl aldehyde. Vitamin A is also delivered in varying doses in all of Environ® products (please contact Environ® or consult our website for further information - see the back of this brochure for details).

Directions

It is essential to thoroughly prepare the skin by topically applying Vitamins A and C prior to PCI-needling. On average, one skin cycle is sufficient to make the changes in the skin necessary to promote healing, improve the resistance of the skin and augment collagen production. The use of the Environ® Ionzyme C-Quence® gel and cream is especially recommended, for a minimum period of three weeks prior to the treatment. Longer preparation periods are associated with faster healing and better results.

The PCI Needling procedure can be performed under local anaesthesia, nerve blocks, conscious sedation or general anaesthetic (or a suitable combination of these) depending on the nature and extent of work required.

PCI Needling is a sterile procedure to be conducted under sterile conditions. Standard surgical consent forms should be signed by the patient and the presence of a nurse or assistant is recommended.

The procedure will take approximately 45 minutes for the facial area. The Environ® surgical Roll-CIT® is available in a number of widths suitable for different treatment areas. Once the procedure has been completed, Environ® EssentiaA Oil should be generously applied. Skin should then be kept moist with saline soaked swabs for between one or two hours.

Instruct your patient to keep the treated area clean (using tepid water and Environ® Sebuwash) and moisturized (using Environ® Debut or a similar Environ® product containing a level of vitamin A suitable to their skin). Skin will have a tendency towards dryness and sensitivity for five to seven days post-treatment. Avoid ingestion of any systemic or topical treatments containing anti-inflammatory drugs as this will reduce the desired effects of the mild inflammatory response which initiates the healing cascade that in turn induces the new collagen and elastin complexes.
